Participation Authorization

I am the parent or legal guardian of the student(s) listed above and give permission for the student(s) to participate in Dance Radiance dance, tumbling, cheer, conditioning, rehearsal, performance, and related activities.

I acknowledge this participation authorization.

Emergency Medical Permission

If I cannot be reached in an emergency, I authorize Dance Radiance personnel to provide reasonable first aid and contact emergency medical services when reasonably believed necessary. I understand that I am responsible for resulting medical costs.

Tuition and Studio-Policy Acknowledgment

I have reviewed the current Tuition & Studio Policies, including the $40 family registration fee, monthly tuition, family discounts, costume fees, payment due dates, late fees, and payment methods. I understand that class placement is subject to enrollment, age, ability, safety, and director approval.

Alternative Dispute Resolution Agreement

To the fullest extent permitted by law, any dispute, claim, or controversy arising out of or relating to enrollment, participation in Dance Radiance programs, services, payments, studio policies, or this registration agreement will first be submitted to good-faith mediation in Weber County, Utah.

If the dispute is not resolved through mediation, the parties agree that the dispute will be resolved by binding arbitration in Weber County, Utah, rather than through a lawsuit before a judge or jury. Arbitration will be conducted in accordance with applicable Utah law, including the Utah Uniform Arbitration Act.

This agreement does not apply to any claim or remedy that applicable law does not permit the parties to resolve through arbitration.

I have read and agree to the Alternative Dispute Resolution Agreement above. I understand that, to the extent permitted by law, covered disputes will first be submitted to mediation and, if unresolved, will be decided by binding arbitration in Utah rather than by a lawsuit before a judge or jury.

Costume Payment Preference

This selection applies to students participating in the annual recital. The costume fee is used exclusively to purchase recital costumes.

Separate Photo and Media Choice
