Registration Terms and Acknowledgments

Registration includes

• Parent or legal guardian contact information
• Emergency-contact information
• Student medical conditions, allergies, injuries, and requested accommodations
• Requested class placement for up to four students
• Permission to participate in dance and tumbling activities
• Emergency first-aid and medical-response authorization
• Acknowledgment of tuition, registration, family-discount, costume, payment, late-fee, attendance, conduct, withdrawal, and studio policies
• Agreement to the Alternative Dispute Resolution process described below
• Costume payment preference for recital participants
• A separate Yes/No photo and media choice
• Parent or legal guardian signature and date

Alternative Dispute Resolution Agreement

To the fullest extent permitted by law, any dispute, claim, or controversy arising out of or relating to enrollment, participation in Dance Radiance programs, services, payments, studio policies, or this registration agreement will first be submitted to good-faith mediation in Weber County, Utah.

If the dispute is not resolved through mediation, the parties agree that the dispute will be resolved by binding arbitration in Weber County, Utah, rather than through a lawsuit before a judge or jury. Arbitration will be conducted in accordance with applicable Utah law, including the Utah Uniform Arbitration Act.

This agreement does not apply to any claim or remedy that applicable law does not permit the parties to resolve through arbitration.

Registration acknowledgment: The parent or legal guardian must affirmatively acknowledge this agreement on the registration form before submitting an electronic signature.
